I (person named above), sign this General Release, Waiver of Liability and Indemnity Agreement ("Agreement") individually and/or as a parent/guardian for minor children listed below, in consideration for celebrating Children and Youth Day (the "Program" I agree that my name or child's name, voice, likeness, and contribution to the Program may be broadcast, copied, and distributed throughout the world, in perpetuity, without limitation, and by any means. I will not receive any compensation for this grant of rights except for the opportunity to participate in the Program. I represent and warrant that any and all material furnished by me for the Program may be used in this way without further obligation to me or any third party. I release Children and Youth Day, its committee members, sponsors, agents, and assigns, from all liability that may arise from any claims by me, or any third-party claiming rights through me, in connection with my participation in the Program. I agree to release, defend, indemnity and hold harmless Children and Youth Day Planning Committee, Pacific American Foundation, Kama'aina Kids, its employees, agents and representatives from all liabilities and claims for any injuries, losses, death, damages or cost (including reasonable attorney's fees) which arise out of or incident to my and/or my child's participation in at Children and Youth Day
I acknowledge that I have read this Agreement and fully understand its contents. I represent that I am authorized to execute this Agreement individually, on behalf of minors in the group, and/or as Guardian/Parents of my/our minor children below. I am aware that this Agreement is a contract to, among other things, release from liability and to indemnity, defend and hold harmless Children and Youth Day and that this agreement is between Children and Youth Day and me individually, and my/our minor children named below.
